Service Coordinator for Peninsula Volunteers Meals Program
NCPHS collaborates with Peninsula Volunteers, Inc. to provide a Service Coordinator (social worker) to assist the participants in their Meals on Wheels program in South San Mateo County. Each day, Meals on Wheels serves 250 older and disabled adults in Southern San Mateo County (Atherton, East Palo Alto, Menlo Park, Portola Valley, Redwood City, San Carlos and Woodside).
In this partnership, the Service Coordinator establishes and maintains relationships with Meals on Wheels participants, identifying those in need of social and health services. The Meals on Wheels driver refers participants to the Service Coordinator, who determines with the participant what specific services might be needed by the individual and coordinates service delivery. The presence of the Service Coordinator dramatically increases the degree of care offered by the Meals on Wheels Program and allows both Peninsula Volunteers and NCPHS to build upon their respective strengths.
